The dungeon is level-locked by default. To access it immediately after install:

Fast-travel: none. Short cut unlocked after solving mid-level puzzle (opens a collapsed stair).

Texture path mismatch. The manual install placed files in the wrong textures/ subfolder. Fix: Go to game_data/textures/dungeon_icons/ . Cut pale_widow_entrance.png and paste it into game_data/textures/overworld/ . Restart the game.
